I will give you a sales receipt for “personal wellness and maintenance visit” however it will not include diagnostic codes and ICD-10 codes, and the visit is non-reimbursable by Medicare or any secondary insurance

  • Reimbursement depends on your particular plan with your particular insurance company. Most companies may also require that you meet an out-of-network deductible, separate from the in-network deductible, before they will compensate. Please call the member information number on the back of your insurance card to find out/know your out-of-network benefits.

  • Pure Symmetry does not submit superbills to individual insurance company, however I will provide a superbill with an ICD-10 and reimbursable CPT procedure codes. I will also support any request for daily progress notes from your private health insurance company as needed.

  • Manual Dry Needling is one of many techniques I utilize in the physical therapy session when I determine it is applicable.

  • No. I believe the EPAT treatment is a tool to utilize within the treatment session. EPAT addresses the soft tissue damage from poor mechanics or chronic postural mal-alignment. As an exception, an acute injury in an otherwise healthy individual may yield a stand-alone treatment.
